What triggers contract extensions?

I've been going through the forums and picking up a lot of good info. But, I was curious about a few things and I was wondering if there was a good summary regarding things like:

What events/changes to my account will trigger a contract extension? And, how long will the extension be for?

If I am past my two year contract (going month to month) will plan changes force a new one year contract?

I thought I've read around here that, after one year, I can buy a new phone using whatever discounts and promotions that VZW is offering for various phones. In doing so, this would start up a new two year contract? Would buying a phone like this after one year affect the eligibility and timing for the NE2 discount?
