8125 - BT headset off, phone thinks its still on

I have a Cingular 8125, ROM upgraded to ver 2.25.11.1 WWE (5/11/06). Installed applications include Voice Command v1.60.4622.0 and I have added A2DP (not sure how to find the version, but I found it here , most recent one at the time).
Everything was working well for the past couple of months. I use a Plantronics 510 while driving, and a Jabra BT620 to listen to music. A few days ago, my phone stopped recognizing when I turned off my headset. Example: I get home, turn off my bluetooth headset, someone calls and I cannot hear them. I notice that the headset icon is displayed - so the phone thinks there is a headset available. It happens when I call out as well. There's a menu option to turn off the headset but it is grayed out.
I've tried resetting the phone, but it keeps happening. I have even removed the devices from the partnership list and added them again. The best remedy for now is to turn off bluetooth when I'm not using a headset, but this is still an issue that shouldn't happen.
I believe it thinks the Plantronics 510 is available because it shows the headset icon - typically when I have the Jabra BT620 turned on, it displays the headphones (music) icon.
It's possible that I had both bluetooth devices on at the same time not long before this began happening. I don't know why that would cause a problem though.
Anybody else experience an issue like this? Any ideas?
