VZW PN-820 Caller ID is Unknown. tried replacing phone

Everytime someone calls me, the phone shows a Unknown call, no number, no name. If I press Ignore, another "call" comes in from the same caller with the correct caller ID. But I cannot accept the call this time.
I have tried to master reset the phone, replace the phone, and called tech support. None helped.
Any ideas?

I installed PhotoContacts software on the phone, and the problem seems to be solved.
