Razr v3m (cdma) from midwest wireless to us cellular

I have a US Cellular account right now with a bad phone that works half the time. I have got a Midwest Wireless (which sold out to Alltel, but when it starts up it says Midwest Wireless (flash screen)) V3M RAZR. I would like to unlock this phone and then lock it for US Cellular. Can anybody please help me on doing this? Thanks!
