Unlocked: "Call failed" for missed calls...?

Whenever I miss a call, I get a "call failed" screen. Nothing technically went wrong, but it will just show "called failed." It happens on my phone and my girl's phone too... Both are unlocked using T-Mobile. 1.1.3

Any ideas? Thanks

(So far, only noticed it happens when we miss calls while we are already on the phone 'call waiting')

I get these as well randomly and have been since 1.0.2. Not sure why. Sometimes I get who the missed call was from and sometimes I don't.
