I've been with T-Mobile for 5 years now and am looking to change providers. T-Mobile has been very good to me, but they lack of lot of features/services I need/want.
What I dislike:
Lack of high speed network (EDGE isn't cutting it)
Lack of decent coverage at home (I often go 20+ minutes without service)
Lack of newer services (Mobile TV, Music, Etc)
What I like:
Excellent Customer Service (best I have ever worked with)
MyFaves (I talk to the same people a lot)
Data Pricing (Unlimited Tethering for $19.99 a month)
I currently have the T-Mobile DASH (Windows Mobile 6.0) I like the phone a lot and would really put it to full use if it didn't feel so sluggish and "bloated".
What I dislike:
Very Slow loading programs, and moving around in the phone.
lack of trackball or wheel
Outlook/E-Mail (It seems very spotty at best and the layout is not organized)
UI (It feels more like a biz phone and not a customer phone)
Multimedia (It just isn't very user friendly when it comes to playing video's and music)
Lack of memory (always having to remove texts/e-mails)
What I like:
Full HTML web browsing (wish it supported zoom)
QWERTY Keyboard (I text A LOT)
Supported Tethering
Ease of downloading/uploading from phone (pictures, music, ringtones)
Software (able to add new software, view PDF, Word, Excel, etc)
I actually had a BlackBerry 8830 for a short period through Sprint and I was impressed with the phone and the EVDO network. However, the 8830 is not the type of phone I'd like to use for personal use. It was an excellent work phone, it handles e-mails excellent (including my yahoo account) but it doesn't have the features I'd like to see as a customer, but all in all, I did like it better than my DASH.
Either way, Can anyone make some suggestions on a provider/phone?
I really like the looks of the Samsung Instinct from Sprint, however, I don't want to join a "sinking ship"
I also really like the iPhone from AT&T, but it lacks IM and only EDGE Support in my area.
I'm not sure about VZW, didn't really see any phones and their prices also seem expensive.

As for provider, you are the only one that can make that choice, especially since you didn't say where you were. You need to find the carrier that has the best service where you will use the phone - where you live, work, travel and play. And don't just believe the coverage maps. Do your homework!
Once you make the carrier choice, then select the phone according to how you will use it.

i am on sprint.. i dont have an instinct obviously sonce its not out yet... but i do have a Q9 and the evdo network with opera mini is fantastic!!!! even with IE on WM6.1 is good! i get 0 to 2 bars at home and evdo still works ! i am typing this from my Q at home 
just so you know the instinct reqires an everything plan.. like the simply everything plan... look it up on sprints site!...
anyways sprint does have other good phones! like the mogul, the touch, the Q9c, the BB's.... anyways... at&t is nice but on the expensive side especially for the iphone.. and verizon is really expensive especially for data! verizons data plans cost a lot! thats one reason i am with sprint! besides the fact that sprint has given me free stuff because i kept getting defective phones... i have free texting... an free TEP and 100 bonus minutes on all 4 lines..
anyways i do recommend sprint for data! and there voice services are good too

Stay away from cdma, those phones all suck. Stick with t-mobile, or go with att and get a nice unlocked/unbranded phone. When you sign the contract with the carrier take the free phone and sell it.
All those carrier services are just gimmicks. For music you can install an internet radio app and listen to any internet radio station in the world, for free. There is also slingplayer if you want to go that route. But youtube works pretty well to kill time and watch various things.
Take a look at some of the high end nokia phones, sony, htc. I'm sure most people will recommend the n95 3 or n95 4 and att. But definitely stay away from the cdma carriers, those phones are just a joke. Not to mention they force certain plans on you based on which phone you have. Like my carrier not knowing what phone I'm using and picking my own plan based on what I need and want.
