WAP with Blitz UTStarcom and Hottproxy

Anyone have any tips for getting Verizon Blitz with UTStarcom with Hottproxy? I had my old Samsung 950A working but manually entered proxy info on phone, not using software.
On UTS... the Data tab, the User Profile MIP NAI I have my 10 digit number @ IP of my Hottproxy server... is this correct? On WAP2 tab, IP is correct and port is 8080 and my router is forwarding port to correct server (local 192.168.15.6). I've also tried turning on DMZ for the server.
Hottproxy has same 10 digit number as user name and default pass.
If I get things running, I'll do some screen shots and post for others. Thanks in advance for any help!
