Lowest price per minute outgoing flat rate global/roaming sim

I've been looking at international/global/world sims which are good in many different countries. I've found 1)They are good wherever gsm services are available. 2)Many countries have free unlimited incoming calls , mostly EU and Middle East. 3) Some have free incoming sms for all countries.
What I HAVEN'T been able to find is a world sim with ONE consistent flat rate for outgoing calls to every country. Would anyone know of such a sim? Is one available in USD , not Euro or GB Pound sterling?
