HELP! Sharp gx22 "PIN Blocked"... "Enter PUK"

I somehow managed to block my PIN?!?! The screen asks me to "enter PUK" and then "enter new PIN" and "repeat new PIN". It then says it's accessing the SIM but returns to "enter PUK"? Does anyone know the default PUK?

I use FIDO for years and I've happened to have the same problem a couple of times as you're experiencing now. What I did was to call FIDO's customer service line and explain my situation. They first asked for my identification and after they've verified that I'm the owner of the SIM card (despite the phone because you can always change whatever phones you like with the same SIM card), they gave me the PUK cold which is quite long, after all that, The SIM is no longer PUK locked and my phone was back to work upon restarting of the phone
