Phone Button was hijacked

Okay, I searched previous posts but could not find anything. My phone button has been altered by Datebk5. I have deleted Datebk5 and it still does not function correctly. Before, it use to show call logs now it just puts me right back to the same phone screen.
Help.

Same thing here. I thought setting all the buttons back to Default in the Prefs would fix that, but it hasn't.
