If the PIN is blocked, you can unlock it with the PUK code. However if you permanently blocked the SIM card and your phone displays PUK Blocked, then you need a new SIM card.
As for the code-related question, I'm not quite sure what you mean. If you want to reply directly within a thread, just click on Post Reply

It sounds like you're asking about unlocking a GSM phone to accept other SIM cards. Depending on the GSM provider and on how long you have been a customer, you can contact the provider directly in order to get the unlock code sent to you via e-mail.
Cingular and T-Mobile used to do this, I'm not sure if both still do this or if only one still offers this method of unlocking a phone. I'm not an expert on this so I can't offer more advice.
As for activating a new GSM phone, you need a new SIM card. With a CDMA phone (CDMA being the network standard that Verizon Wireless, Sprint, Alltel, US Cellular etc use), the provider assigns the phone's ESN (Electronic Serial Number) to their network as a way to authenticate the user access for that network.
But as for telling you how to unlock your phone on your own, that's something which we can't discuss out here in the open forum due to the forum rules.
