This usually happens when in one or more of the following situations is true:
1. Your USB port has gone bad and/or loose
2. If front USB ports, the cable has become disconnected from the motherboard and/or was connected improperly.
3. The USB device is fried and defective
4. Windows drivers somehow corrupted (see explanation below)
I know that I once accidentally connected the front USB header incorrectly to the motherboard and I fried two USB devices. One was just a USB to PS/2 mouse converter but the other was a wallet size 512MB USB flash drive
Try using other USB ports or cleaning the contacts on your cable before you go jabbing it in and out too much more.
The Windows corruption I encountered before is very strange. I tried reloading windows again, etc, and still the device was showing in device manager as VEN_0000 and DEV_0000. On other computers it was fine and after reloading windows completely, it worked fine.
Check to see what Windows is detecting the Vendor and Device ID numbers as by doing the following: (assuming you have Windows 2000/XP)
1. Click Start -> Run -> type "WINMSD" without quotes, Click OK
2. Expand "Components"
3. Click "Problem Devices"
4. Look under PNP Device ID and see what the ID string is. Try different USB ports and refresh the data and see if it's the same.
Let me know what you find!

The problem I usually have with this error is dirty contacts on your data port. I work with many many phones every week and that is the main cause of errors that I receive when connecting a phone. Try using a toothbrush with some isopropyl alcohol and clean your data port. Isopropyl alcohol works well and will not cause corrosion on your components. Usually after cleaning the phone will detect fine. Let me know if you get it working.
