V3 Flashing error - please help

Hopefully someone out there can help me out - I was trying to flash/flex my V3 with a monsterpack, but am having some issues. I bought my V3 in the UK, had it unlocked and am now using it with T-Mobile US. I first followed the guide here: http://pagefusion.com/portals/25/index.php but RSD Lite 2.3 gives me the following error when I try to flash: "Failed flashing process. Error getting subscriber unit information. Device API Error: 0xE003003B Command: RQHW (0xE083203B)"

So then I tried using PST MultiFlashFlex Tool 1.8 and I get the following error: "Error getting subscriber unit information. Error 0xE003003B" Now my phone is sitting here with "SW Update in Progress..." on the screen and I don't know what to do.

I tried flashing both R374_G_0E.40.9CR_PDS004_LP0003_DRM0003_JPJAVA_G_0E _V3_04_05R_GSMV3xxT674TA09E_1FF_MOTOX.shx
and
R374_G_0E.42.0ER_PDS005_LP0003_DRM0001_JPJAVA_G_0E _V3_01_0AR_SE8617MXXF1126_1FF.shx
with the same results. Thanks in advance for your help.
