The problem was that even though I had deleted every contact on the phone first, when I did a bi-directional sync it must have restored the phone setup from a backup and then combined the two on both the phone and in my Outlook Phone Contacts folder. The solution is to edit the Outlook folder however you want it to be, set the sync direction to OUTLOOK -> PHONE only and re-sync. Now if only the Outlook categories would carry over to the Nokia groups, I'd be all set.
