Just unzip and place this to c:\windows\system32\drivers.
After doing this connect your phone.

this is an OLD driver... you're better off using -> http://mark.cdmaforums.com/Files/DriverTool-421a.exe

The driver attached in the original post of THIS thread is...

USBMOT2000.INF
DriverVer=1/10/2005, 2.0.0.4
