Some of the agreements have MTM 1 2 and 3 year agreements. Depends on the company and code.

just out of curiosity, where did you get all these details about your corporate plan including the MSD codes.
I have signed up for a corporate plan from the hospital where I work and never received all these info.

You can find your MSD code on the bottom right hand side of page 1 of your bill.
The rest of it should be on your actual contract paperwork.

You're right!! I never notice these four letters underneath the box where you fill out the "amount paid".
My MSD code is NBME for Sunnybrook HSC but I don't have any details for all the plans aside from the one I have now, that's available to us. There's nothing about Rogers Corp plan from our corporate intranet.
