Previous phone was i870 with contacts saved to a *.mdb file (access)
through Iden phone book manager. Can anything be done to port these contacts to instinct using outlook or any other program.

actually I have both
The short answer is yes. I had to jump through some hoops to get it to work for me. Here is what I believe you will need to do. Use Excel to open the mdb file and then save it as a csv file. Import that file into Outlook. When you do this you will have the opportunity to assign the proper Outlook field names to the fields in the csv file. Once this is done and you have the contacts in Outlook you will then export the contacts to a DOS (i think) based csv file. Now, log into your mysprint account and through the mobile sync page you will have the option to import contacts. If the DOS based csv file doesn't work try the Windows based csv file.
Since you still have the old phone, I think a Sprint store can transfer the contacts for you. I've seen the go from a sim card to a sim card, and from my previous iden phone to the instinct. Sorry, I should have asked if you had the old sim card, too.
