MMS isn't up for me yet in Dallas, TX. I'm using a non-jailbroken iPhone 3G with OS 3.1 and at&t 5.0 carrier file. I've also had both a 2G and 3G iPhone since the days each came out. Also, I'm on a family plan with a FAN discount and unlimited texting only for my line.
Looks like it's just a customer-by-customer basis aka a staggered release...

Yea... I had those settings and it didn't work... then I added the username and password... still doesn't work... I even tried the ":80" at the end of the MMS Proxy string.. and nothing... oh well.. might have to wait until the 25th

Have you tried putting the iPhone sim in a non-iPhone phone?
Make couple of call from the non-iPhone, then turn the phone off and on...make couple more calls, then put the sim back on the iPhone. Also reboot the iPhone after you load the 5.1 carrier file
MMS is working pretty good for me here in Jersey. I can send pictures and video and receive both as well. The one weird thing is: my GFs blackberry won't receive pictures from my phone but I can get pics from her.
I'm on 3.1 I did the same thing as COF23 and my setting are the same. Updated the carrier file via iTunes 9.
On a side note: The iphone downgrades the video so bad it's almost not worth sending. It looks like less than QVGA at 10fps. My friend with a Rogers HTC magic rooted with the Hero firmware sent me a video and it looks great.
