Anyway, I hope this tutorial will help you on how to make your own custom "themes" for your SH-06A and other models!

Photoshop or any other good programs. Maybe Gimp? (haven't use it before)

Methods to make your own menu screen Difference between 2 methods
The difference in this two method (JPG and GIF) is quite big.
JPG Pros
Images can be more fanciful.
Images are of much better quality.
JPG Cons
Background cannot be change as and when. It has to be the one that you set.
Icons are not animated.
GIF Pros
Background can be changed anytime as icons have a transparent background.
Icons can be animated
GIF Cons
Images quality are quite bad.
Design cannot be as fanciful as JPG due to transparency restrictions.

Therefore, personally I prefer JPG method.
JPG Method
The JPG method is only applicable to those whose screen is 480x854 and whose icons are 152x152.
And I think sharp models only (not very sure about this)
1. Replace the background layer with your own background.
2. Hide (press the eye) the Margins folder (it's just a guideline for you to know where your icon should start)
3. Hide the Selected Icons Folder.

Making your icon
4. Select an icon folder, press the down arrow.
For example, in i-mode: http://www.flickr.com/photos/hardbread/3935758535/
5. Hide the imode layer: http://www.flickr.com/photos/hardbread/3936549754/
6. Paste your icon (as a new layer) on top of the respective image layer.
To get the icon in the center of the square, ctrl + left click i-mode then press ctrl + v.

7. Repeat for all the other icons.
8. Once all 12 icons is completed, it should look like a real menu screen.
* Remember to save! Save as psd file to keep your layers.

Making your Selected Icon Image
9. Hide your Icon Folder
10. Show your Selected Icon Folder
11.It's the same as making your icons. Just that you do the layers in the Selected Icon Folder.
Saving the Icon Images
12. Ctrl + left click imode layer (the red square)

13. Go to Edit > Copy Merged
14. Go to File > New > Width:152 , Height: 152 (should be already pre-set) > Ok
15. Ctrl + V
16. Go to File > Save as > DSCN0001 (make sure it's JPG) > Save > There should be a prompt, just press OK.
17. Repeat with all your other icons and you're done!
Making the Background
18. All layers/folder hidden except Background and Icon Folder.
19. Go to File > Save as > DSCN0026 (make sure it's JPG) > Save > There should be a prompt, just press OK.

Non-Photoshop Users

Replace the coloured squares with your icons and the background with your own.
The steps should be somewhat like the photoshop tutorial.
GIF Method
This is the easiest way to do.
All Icons should just be 152x152.
Background should be transparent (depending on how you want it to be)
Each icon should have 2 different images. One for icon when selected and when icon is not selected.
Save both separately as GIF and upload!
The name for GIF to save is STILXXXX (XXXX being numbers).
That's all!

3. How to make your Outgoing/Incoming Calls, Sending/Rcv Messages
For this, there is no restriction as to what your file size should be.
But I recommend 480x288.
I use ImageReady to animate the images.
There are 4 Images which you can edit.
Outgoing Call

Sending Message
Receiving Message
Save them as STILXXXX.gif (XXXX being numbers)
Thats's about it.
4. How to make your animated notices
There is a restricted file size for this.
Dimensions has to be 424x84 or it will not be read.
Save it as STILXXXX.gif as well.

5. How to upload them into your phone
JPG FILES
1. Connect your phone to your PC. -> MicroSD mode.
2. Create a new folder in your DCIM. Name it 100NIKON.
3. Copy all DSCN images (0001 to 0025).
4. Paste into your 100NIKON.
5. Disconnect your phone from your PC.

1. Connect your phone to your PC. -> MicroSD mode.
2. Go to PRIVATE > DOCOMO > STILL
If you do not have a STILL folder, create one.
3. Paste your STILXXXX.gif images into STILL folder
4. Disconnect your phone from your PC.

6. How to move from your Microsd to your phone
JPG FILES
1. Menu > Data Box > My Pictures > Microsd > 100NIKON
2. Images should all be there
3. Click Submenu > Move/Copy > Copy to the phone > All in folder
4. There will be a security code prompt. Code is 0000
5. A list of folder would appear. Choose any available (in this case I will use the folder name as Acquired Data) then press Fix.

Hey, I'm not able to edit my post.
And I need to make some correction. I'm not sure if it works on SH01/02/03 due to the alignment problem.
The margins may be slightly different.
Hence, the gif method will definitely work on the other models. For now, JPG method only with SH06A and SH07A.

Made a new menu screen (Apple). Compatible with SH06A and SH07A for now only.
