Well it's unpleasant to lose your phone, or any other thing that is useful for you and especially if it's not very cheap but I wouldn't turn this into a fobia.

It's more than a few hundred dollars lost. It's losing all your important stuff on it, as well. That stuff is priceless and no amount of insurance is going to recover it.
Unlike, Kelly Reeves I don't place my devices near the sink or other body of water. In that case, I leave it well away from any body of water or tuck it deep in my pockets (or stash it in some other safe spot) so it doesn't take a dive into the toilet. If there's any risk of damage to it, I will usually do without them. Breaking/damaging them is just as bad, as having the phone being swiped from you.
As for checking my phone multiple times a day, I can only do that on my breaks or lunch, outside of those two scenarios I'm not allowed to have my personal cell phone on me. I see this rule being broken by a lot people, but I do my best to stay within the rules.
I do, however check my pockets/backpack frequently to make sure my phone hasn't jumped out or somebody hasn't slipped their hands into the pocket/backpack.
Yes, jacked my BB900 a few years ago, my fault but I was PO.

I usually don't invest in insurance, but that's why I do pay for the full coverage insurance with my carrier. That way aside from dropping it, even if the phone "disappears", for a deductible of something around $120 I can get a replacement of the same model (or similar), as opposed to shelling out $550 "suggested" retail price. If someone stole my phone and I caught up with them, their medical bills will be FAR more than that.
I love playing with my apps and tech on the phone, but I wouldn't go nuts if I couldn't. I have a prepaid I use for other reasons on the side when I don't want my real number given out (Ebay, etc) and if I HAD to use that for a while as my primary phone, it wouldn't freak me out into a straight jacket.
I'm most afraid of losing mine because my computer is too old to run the current version of BB desktop software I would need to back it all up and I'm not in a spot budgetarily at the moment to buy a new one.

Knock on wood, but I have never lost a cell phone in my whole entire life. That would be a costly and disappointing experience if it ever happened to me! I always know the location of my phone, as it almost has become an extension of my body lol.
I always have a system where I say "Phone, keys, wallet". As long as I have those three items, I am good to go! Once I confirm this check, I know I have everything I need lol.
havent lost my phone yet, but i highly depend on it especially my blackberry
without phone these days will be hard. knowing that you can easily use phone anytime and everywhere, knowing that life is easier with phones.
