Port out of h2owireless?

What is my account info from h2owireless or how do I obtain the necessary info to port out to simplemobile?

h2o account number = SIM number
h2o account pin = last for digits of SIM number

Now, trying to port can be a royal pain. Took me 6 weeks to port from h2o to Tru. Others have had less of a problem so I am guessing the issue was more on the Tru side.
