Why some iphone 4s verizon/sprint have Banner+Signal Bar, and some only have signal bar only and no banner?

While I can't say for certain, it may differ market to market or could be related to the version of iOS installed in the iPhone. Maybe someone else can offer a better explanation.

Only time I've ever seen them without the banner is on commercials because they don't want to show what carrier it's on (as that'd be bias and there are so many different carriers now days.)

Usually the lack of carrier name/banner on CDMA networks (Verizon/Sprint) means that the iPhone is not registered on the network, usually due to an outdated PRL.

I have 2 friends with Verizon iPhone 4's who both had their carrier names disappear one day and a quick reset of network settings brought it back.
