Fees and Taxes when getting a new iPhone

Heres my situation:

Currently:
I currently have a iphone 4 32gb on Verizon i am only on my second month with it on Verizon. I started a no contract month to month service. And my account currently says upgrade discount eligible.

What I want:
When the new iPhone comes out this month, next month, October or whenever I want to buy it on contract.
And if apple holds with what they have been doing I plan on getting the top of the line one which is currently $399.
+
$99 apple care + plan or what ever it is because If I am going to get into a contract for 2 years I am going to get the best iphone and have 2 years of warranty.
= about $500

Questions:

What type of taxes and fees should I be expecting when I buy the iphone with the apple care at Verizon or Apple Store online?

What type of Fees and taxes should I expect If I reserve it online at Apple store or Verizon?

I heard there is a Verizon upgrade fee so what should I expect for fees from Verizon when I get on a 2 year contract with the top of the line iphone?

What total cost should I expect?

(I live in Kansas for Taxes and what not)

Verizon charges a $30 upgrade fee. Taxes on the phone are only charged when you pick up the phone, not when you pre-order with a deposit, based on your local tax rate (state/local). Hard to give you an accurate estimate unless someone lives in the same area and has recently purchased a phone of the same price you are thinking of. At this point, the next iPhone's pricing has not been determined. Hope that helps somewhat.
