Was the number working on the prepaid provider once you transferred it over from Monthly 4G? If not, the issue may lie with the prepaid provider.
Generally one would have it working on that carrier before doing the port back to T-Mobile.

So I went from a Monthly4G plan to a Value plan 10GB, in order to do this the rep. at the store said I should port my number away from T-Mobile to a prepaid provider and then back to the contract plan.
I did this process within a day and its really screwed up right now. Now it's giving people a not in service message when calling my ported number, and my T-Mobile phone still show's the temp. number. Has it gotten stuck in my old prepaid account where the number originally was?
Have I lost the number permanently since it says not in service?

I recently had a similar problem on reverse - Tmobile owned MVNO to Tmobile. Tmobile now outsources their porting department to the Philippines. Great people who seem to have no training regarding porting.
Actually you did not need to port at all. They can transfer a phone number from prepaid to postpaid or vice versa but you must ASK them to transfer. Porting only applies to transfers between two different companies. Don't make the mistake I did and mention the word porting.
If they won't correct the problem PM me and I will put you in touch with someone at Tmobile who will help you.
OP- I went the same route as you. But actually, my number originated on a T-Mobile family plan and I was trying to port to my own T-Mobile account. It was suggested to me that I port out to a prepaid carrier first, then port back into T-Mobile. I was without a phone for about a day, and yes, callers did receive a "number not in service" message during the port.
