technically no. You cannot move to the $30/5GB plan. It's for NEW ACTIVATIONS only.

I will just order another SIM, then. Just in case. I use Google Voice exclusively, so they can keep the old number.
Just stay on the $30 plan. You can use Sipdroid over 3g to make your phone calls.
Once you move off the $30 100 mins plan the option to switch back to the same plan will disappear. Only the other $30 1500 mins/text + 30MB Web will show up.
I just got activated the $30 plan on a Nexus 4 with a sim card that I bought on T Mobile's website for 99 cents. I spend a lot of time in NH during the summer months where there is no T Mobile service. Could I switch to Straight Talk (with an AT&T compatible sim) for the summer and then go back to T Mobile's $30 plan in the fall if I buy another T Mobile sim card?
Yes you can
Would it be possible to get a sim at a local T-Mo store and get on this plan? Or is it buy a phone from walmart or a sim on tmobile.com only?
You can use a SIM from a T-Mobile store, but most stores won't sell just a SIM. They're an OK place to get a SIM if you don't mind buying a phone at the same time. You have to activate the SIM through the web site, though.
Yes.
If your local store has a prepaid SIM kit available to sell you (which includes a SIM activation code), then it'll work fine. Activate the SIM on T-Mobile's website under the prepaid section. During the activation process you'll have the opportunity to choose $30 plan under the monthly plans.

Be aware there are reports here on HoFo of some T-Mobile stores not carrying prepaid SIM kits, or if they do, refusing to sell them if a prepaid plan wasn't purchased in the store (which you can't do with the $30 plan). One of my local T-Mobile stores had no problem selling me a couple of prepaid SIM kits after I even told them I wanted them for the $30 plan.

I tried at the mall today and had no problem purchasing an activation kit w/ micro sim. So yea. If one location in your area won't sell you the kit, it's quite possible another will.
Just remember, any time you switch off, there's always the risk they'll cease offering the $30 plan at all before you come back.
