It's a 4, cdma version has no sim card slot, you're stuck other than flashing to cricket (not recommended and/or not legal) or selling on ebay to recover some of your money, I've learned to always check the esn or imei of a phone by calling the service provider before hand, better safe than sorry from now on, good luck
Seriously, harass their customer support, you are a paying customer. Write a mail to dan.hesse[at]sprint.com and complain that you don't understand why you as a paying customer can't use your new iPhone on their network. Make the pain big for them
I would still try to get them to hook it up. If I was the carrier I would gladly hook up the device in order to recoup the money, so I've never quite understood the carriers not hooking up phones after the last customer fails to pay. Seems like they would rather recoup that money somehow

If the carriers activated those phones, fraud would go thru the roof. Buy a phone on contract in cash, sell it, never pay anything, and claim identity theft in a few months to restore your credit. Happens all the time. In this case, sprint will collect money whether they activate that phone or not, being as the customer appears to be under contract. No reason for them to help a fraudster.
