The image that you're showing there is LTE low and high bands with MIMO. Which takes only 1 antenna. That leaves two other antennas. I know the manufacturers that AT&T uses but am not sure if that is protected under NDAs that I've signed. I know this the manufacturer and model are reported to the FCC but am not sure if its in the public domain. Why would there not be any CDMA/WCDMA antennas on this site? AT&T usues VoLTE only in certain markets and GSM is in the process of being decommed so you would more than likely have 3G UMTS(WCDMA). As for phantom, all of the carriers will build the towers to look like palm trees if it is necessary to obtain leasing.

There is WCDMA on the site, I thought you were referring to CDMA 1xRTT.

Right now only low band 700Mhz 5X5 LTE is broadcasting from this site.

From the looks of this site its 700 MHz LTE w/ 2x2 MIMO and PCS or AWS in a 2x4 MIMO. Can't tell just from the cabling shown which band or the block bandwidth, but I'll take your word that they're running on 5 MHz blocks.
